（4） Travel Purposes (Tourism, Visiting friends or relatives, Business Affairs etc.)
（5） Scheduled Travel Dates
（6） Intended length of stay in Japan
Note : If the application procedure is not completed within the appointment time due to tardiness or incomplete paperwork, application will not be accepted
